右美托咪定改善围手术期睡眠质量的临床研究进展

围手术期睡眠障碍是临床麻醉与围术期医学里常见的难题，它的诱发机制涉及手术创伤应激、麻醉药物残留影响、环境变化以及患者自身心理状态等多个方面，情况比较复杂。睡眠结构出现紊乱后，会让患者术后疲劳感加重、认知能力下降，还会提升术后谵妄、心血管不良事件及慢性疼痛的发生概率。当前，围手术期睡眠管理主要依靠环境干预和传统镇静催眠药物，这些方法存在疗效有限、易引发药物依赖及呼吸抑制风险等比较大的局限，没法满足加速康复外科对睡眠质量精细化管理的要求。本文重点放在高选择性α2-肾上腺素能受体激动剂右美托咪定上，目的是系统梳理并评述它在围手术期不同阶段（术前、术中及术后）对睡眠结构、睡眠节律及睡眠深度的调控作用，探讨它区别于传统镇静药物的独特药理学优势。

Perioperative sleep disorder is a common problem in clinical anesthesia and perioperative medicine. Its inducing mech-anism
involves many aspects, such as surgical trauma stress, the in?uence of narcotic drug residues, environmental changes and the patient's own psychological state. After sleep structure disorder, patients' postoperative fatigue will increase, cognitive ability will decline, and the probability of postoperative delirium, cardiovascular adverse events and chronic pain will be increased. At present, perioperative sleep management mainly depends on environmental intervention and traditional sedative hypnotic drugs. These meth-ods
have limitations such as limited e?cacy, easy to cause drug dependence and respiratory depression risk, and can not meet the requirements of accelerated rehabilitation surgery for ?ne management of sleep quality. This article focuses on dexmedetomidine, a highly selective α 2-adrenergic receptor agonist, in order to systematically review its regulatory e?ects on sleep structure, sleep rhythm and sleep depth at di?erent stages of perioperative period (preoperative, intraoperative and postoperative), and to explore its unique pharmacological advantages di?erent from traditional sedatives.
